Pre-Employment Forms
IMPORTANT: Employees MAY NOT begin working until the criminal background check and sex offender registry check have been completed. The supervisor must receive notification from HR of the completion of both checks.
New & Returning Employees
For new employees and returning/retiree employees if separated from DMACC for at least one year. The following pre-employment forms must be completed and approved by Human Resources:
- Request for Adjunct, Temporary or Student Employee (P-61A)
- Authorization/Consent & Release for Background Check (P-61B)
- Data Record for Regular Employees (P-13) or Data Record for Credit Adjunct Instructors, Temporary & Student Employees (P-14). A Data Record is not required for Non-Credit Adjunct Instructors.
- Course Approval for Instructors (P-24) (adjunct instructors only)
- Employee application and transcripts (full time employees and adjunct instructors only)
