entrance requirementsThe following items must be completed before you attend the first day of class:

  1. Attend an informational session with a program specialist. These are held each Tuesday and Thursday evening at 5 pm at 2081 NE 54th Ave. in Des Moines. Spouse and family members are encouraged to attend this meeting as well. Note:  If you're out of state, call 1-800-35-DRIVE to get an information packet.
  2. Be at least 18 years of age.
  3. Obtain a copy of Motor Vehicle Record. You can get this from the D.O.T - cost is approximated at $5.50. (need before first day of class)
  4. Pass a D.O.T. physical (do not take drug screen, one will be given to you during the first week of class.) You may get a D.O.T. physical from your personal physician--obtain a D.O.T. physical from from us to take with you.  Or you may go to Concentra Medial Centers, 2100 Dixon, Suite E, Des Moines, IA 50316, 515-265-1020.  Typical physical costs range from $50 to $60 and they already have the D.O.T. physical forms.
  5. Present your actual Social Security Card.  If you do not have the actual card, one can be obtained free of charge at the Federal Building and you WILL need this before you are allowed to apply for your permit and license. Laminated cards are not accepted by the D.O.T. (new D.O.T. rule).
