Referral Checklist & Forms for STRIVE

REFERRAL CHECKLIST (For special education teacher use only)

All forms listed below must be included before a student referral will be accepted.  Attach this sheet to the front of the following information and send to:

_____ Completed and signed Student Referral Form (click here to download form*)

_____ Completed DMACC Application form (click here to download  form*)

_____ Copy of student's junior and senior IEP (The IEP will need to be updated prior to the student attending STRIVE. Academic goals are to be dropped and a vocational goal must be put in place. It is not necessary to send this IEP. We can access it online.)

_____ Copies of Three-Year Evaluation

_____ Copy of Evaluation Reports

_____ Copies of results of interest inventories, exploration experiences, and   
                other pertinent information

_____ Copy of up-to-date High School Transcript
