Nursing students need to complete and submit the Student Health and Immunization Record when beginning their program. Click below to see the form for your campus.

The form must be thoroughly completed with signed physician verification of current immunization, conditions requiring treatment, and/or special accommodation. At their own expense, students must have obtained the required physical examination, immunizations, and/or tests prior to beginning the first term, or as requested by the Program Chair. Existing health problems must be identified. Students will not be permitted to go to practicum areas until required health forms are completed and submitted.
- Changes in health status after completing health forms must be reported to the Program Chair.
- Students are to make a personal copy of the history and immunization forms prior to turning them in, as this documentation is frequently requested by employers throughout one’s nursing career.
- Infectious diseases, including Hepatitis B Virus (HBV), Varicella (Chicken Pox), Tuberculosis, Measles, Mumps and Rubella may have serious implications for both health care workers and clients. Therefore, students are required to meet current standards for prevention and screening, unless a signed medical waiver is submitted.
Students are required to inform their clinical and lab instructor of any condition/disease/medication that may relate to precautions being taken to safeguard the student and/or client and abide by agency policy.
It is strongly recommended that students obtain health insurance. A student health insurance plan is available through Student Services.
