Going Home Reentry Program - Frequently Asked Questions

What is the role of a Case Manager?

The case manager begins working with offenders during the incarceration phase to comprehensively assess interests, aptitudes, skills, and disabilities; analyze labor market information; and develop a career plan. During the transition phase, case managers work with area employers and educators to place each offender in a career and/or educational path, while assisting in the arrangement of formal support services such as housing, employment training, substance abuse treatment, family counseling, transportation and childcare.

Can offenders in institutions other than ICIW, Newton and Fort Dodge participate in the program?

Yes. All eligible offenders (inmates between the ages of 18 and 35 with an LSI-R score between 14-40 who's expected to be released to Polk County within nine months of the start of a new program) can be transferred to one of the three specified institutions for participation in the program.

What if I'm not being released to Polk County, can I still participate?

Those meeting all other criteria that are not being released to Polk County may participate in Phase One ONLY. In order to receive full services of the program, you must meet all eligibility criteria.

I'm an ex-offender and I've already been released, can I participate?

Case manages are available to help you find resources specific to ex-offenders. While you won't be able to take full advantage of the program's services, you can contact us for support.
