This will be the final practicum course in the respiratory therapy program.  You will continue to be involved in all procedures from the previous practicum classes.  These include care of persons in adult, pediatric and neonatal intensive care areas, participation in CPR, observation of cardiopulmonary diagnostics and general patient care techniques.  When available you will be rotated to view autopsies and cardiothoracic surgery.  Some of you will rotate to a variety of secondary rotations.  It is your last opportunity to gain experience in areas or with procedures from which you feel you would benefit.  Please let your clinical instructors know of any procedures or areas in which you want to spend time this term.  You will have more freedom to determine your assignments in this term than your previous practica.

 

With ventilator care ALL VENTILATOR CHANGES WILL BE COMPLETED WITH THE CLINICAL INSTRUCTOR INFORMED AND CONSULTED PRIOR TO ANY VENTILATOR CHANGES. 

All documentation of checks or changes on ventilators must be countersigned by your clinical instructor/supervisor.

 

The attendance policy continues to be that all missed clinical time must be made up prior to the end of the term.  Each day of missed clinical time will result in a reduction of the class grade by 3%.  All lost credit may be regained if: 1) The reason for missing was acceptable, 2) The clinical site was notified before the scheduled start time, 3) The sick day is documented on DataArc within three days and 4) The time is made up.  Time missed because of arriving late or leaving early will be made up at the rate of one (1) hour for each 15-minute increment.  If you miss more than two days of practicum, we cannot assure you will be finished prior to the time grades must be turned in.  In that case, you must complete a contract for an incomplete grade for the course.  In the absence of this contract, the only option is to assign you a grade of F.

 

RESOURCE MATERIALS:  You will be using books from all previous classes in the respiratory therapy program.

You must complete all items necessary for a passing grade to be received in the class.

 

1.         PERFORMANCE EVALUATIONS:  You will be evaluated by your instructor(s) at each facility during the term.  This term we will use a new form and method for evaluation.  You will have a short evaluation completed for each day of practicum.  You will be provided the forms on the first class day of the term.  This evaluation will be submitted directly to Kerry George after each clinical day.  You should provide the evaluation form to your instructor at the beginning of each day.  The instructor will complete the form and provide it to you at the completion of that day.  Performance evaluations are to be turned in directly to your instructor.  Be certain the evaluation contains both your signature and the signature of the instructor.

 

2.         RRT MULTIPLE-CHOICE SELF-ASSESSMENT EXAMINATION:  On June 17 you will take a self assessment version of the NBRC Written Registry Examination.  This is a 100 item multiple choice examination.  You must achieve a scaled score of 70 on this examination.  If you do not pass the examination you will be given additional opportunities to pass a version of the examination prior to the completion of the term.  Your score on your first attempt will be used in determining your course grade. 

 

3.         RRT CLINICAL SIMULATION SELF-ASSESSMENT EXAMINATION:  On July 6 you will complete a self-assessment version of the clinical simulation examination.  You must achieve a scaled score of 70 on both the Information Gathering and Decision Making portions of the ten-simulation test.  You must complete the following 5 simulations on the DMACC network in preparation for this examination.  The simulation must be completed at the Advanced Difficulty Level and the cover sheet with your scores submitted to Kerry.

4.         COMPETENCY EVALUATIONS:  During the term you will be required to be evaluated on your performance of certain procedures.  You must complete the required competencies for the current term as well as any that have carried over from a previous term.  A list of any competencies carried over is posted outside Kerry's office.

            Each competency not completed will be averaged with the others as a zero.  You will be expected to complete the checkoffs if you do the procedure at any time during the term.  The competency assessment must be in the DataArc site within seven (7) days of the date on which you complete it.

 

5.         CARE EVALUATIONS:  You will be required to complete two summaries/evaluations of care provided to persons on mechanical ventilators.  These will be done on the forms that have been used on the past.  The persons you select will have been on a ventilator for at least 72 hours and have at least three changes (no FIO2 only changes) preferably documented by blood gases within 2 hours after the change following the initial set-up.  The first will be due on the date you will lead a discussion of your care summaries in class.  The other will be due by July 20. 6.         Review Presentations:  Groups of students will present a review session about the items contained in the NBRC Content Matrix as designated below.  Following discussion with your group and the instructor, you will develop a presentation to provide the class with a review of information that will assist them to answer the types of questions that may be asked on the CRT and RRT examinations in the assigned area of the matrix.  In addition to a presentation to the class, the group will prepare some type of information to be distributed to the class.  The information shall be in an electronic form.  The written information shall be submitted to Kerry and then be posted to the course web site.  You may also distribute a printed copy to the class before or at the time of presentation.

7.         PHYSICIAN INTERACTION:  Students will be required to complete at least 12 points of physician interaction during the term.  This time must be documented in the section of the daily log on the DataArc database.  The name of the physician(s), type of interaction and the duration must be included each day.  There must be something in each of these blanks for each day you attend clinical this term.

 

8.         HEALTH EDUCATION:  Each student must document participation in activities designed to educate the public about respiratory health.  A minimum of one hour with specific learning objectives will be needed to meet this requirement.  This activity must be completed during the summer term for full credit to be received.  If the activity was performed before this term, you will receive 80% credit for the activity.

 

9.         ELECTRONIC TIME SHEETS will be completed on DataArc for each day you are in the practicum site.  You should log into the system at the start and log out at the time you have finished providing report to the oncoming shift.  It is your responsibility to assure that the time sheet is completed for each day.  If there is a delay logging into or out of the system, it is your responsibility to enter a time record at some time that day or the next and provide the correct information, in writing and signed by your clinical instructor for the day to Kerry within 7 days of the date for which the information needs changed.

            You must enter some information on the start and end times so your classroom instructor can modify the record.  You must complete a daily log sheet for each day you are in practicum.  These are to be completed on the DataArc.  At the completion of each clinical day, you will document those things you did that day and have the information validated by your clinical instructor/supervisor.  If you have an instructor who is not on the list of instructors, you should call Kerry or Larry within that clinical day to have that person added to the list of instructors.  It is your responsibility to assure your instructor validates the sheet you enter.  If the instructor cannot access the site, a written list of the items you performed that day, signed by the instructor, can be submitted to Kerry for validation.  The DataArc site will be checked every Friday.  Items dated through the preceding Thursday must be on the site and any corrections needed must be to me in writing by that time.  If any item is not entered into the site by this time, it must be redone.  This includes making up the day if there is no time record or daily log for that day.  Any daily log that does not have information in both the physician contact and significant experience boxes will be treated as incomplete and the day will be made up at the end of the term.

 

            All competencies must be entered into the DataArc system as well.  It is your responsibility to assure your instructor submits these for all check-offs they perform with you during the term.  If an instructor is not able to submit the competencies, they must sign a printed copy of the competency.  You can complete and print the form from your access to DataArc.

 

            For ventilator parameter and mode changes the instructor will complete the generic adult or pediatric competency and identify the type of change in the Title box of the competency.

 

            Any competencies not entered by one week after they were performed, will not be accepted.
